Central
Piedmont Health Services, Inc (CPHS) three healthcare
facilities, Sandy River Medical Center, Martinsville Family
Medical Center, and Southside Family Medical Center, are
designated Community Based Outpatient Clinics (CBOC’s) for
area Veterans.
CPHS
proudly serves those who have serves us; area Veterans in need
of primary healthcare.
CPHS
staff, Karen Plum, Director of CBOC operations, and Sharon Karr,
CBOC Coordinator, work directly with the veterans to assist them
with any concerns or questions. This more direct, personalized
attention is for the betterment of the staff and veteran
patient.
Veteran
Guidelines
In
order to receive primary healthcare services, veterans must:

Be assigned to
the Danville, Axton, or Martinsville, Virginia CBOC sites.

Be seen at one
of the clinics at least twice a year.

Update his/her
financial information (MEANS TEST) yearly.

Plum is at Sandy River Medical Center Monday’s, Thursday’s
and Friday’s, Southside Family Medical in Danville,
Wednesday’s from 9:00 am to 1:00 pm, and American Legion Post
1097 located on Fairfield Avenue in Danville from 3:00 pm to
5:00 pm on Wednesday’s, and Tuesday’s from 9:00 am to 4:00
pm at Martinsville Family Medical Center. |
